Matthew Prather Guitar LA Rocks Summer Camp Hold Harmless Agreement
LA Rocks Summer Camp, being held through Matt Prather Guitar, strives to provide a summer camp program that is safe for all participants. However, parents/guardians of participants are encouraged to understand that there may be risk of injury in engaging in camp activities. LA Rocks Summer Camp will focus on reducing any such risks through encouraging all participants to follow the rules, instructions, and conduct codes of the camp.
Matt Prather Guitar does not carry medical insurance for participants. Each participant that registers is required to review their own health insurance policy for coverage. If a participant does not have health insurance, Matt Prather Guitar and LA Rocks Summer Camp will not be held responsible for any injuries or medical costs sustained during participation in LA Rocks Summer Camp. You must sign this Waiver and Release to participate in LA Rocks Summer Camp. Please read through carefully and sign to acknowledge that you will not hold Matt Prather Guitar or LA Rocks Summer Camp responsible for any possible injuries sustained during participation in the summer camp.
Release of Claims
By signing this form, I agree to hold harmless Matt Prather Guitar, LA Rocks Summer Camp,
and any employees or volunteers of the LA Rocks Summer Camp, in the unlikely event of
injury, including death, damages to self or personal belongings, or loss regardless
of severity, that may arise through my child’s participation in the LA Rocks Summer
Camp. I agree to assume full responsibility for any medical charges that may arise
in the event of injury, illness, damages, or death my child may sustain as a result
of participating in the LA Rocks Summer Camp.
I fully release and discharge Matt Prather Guitar, LA Rocks Summer Camps, and any associated employees or volunteers, from any and all claims resulting from any possible loss, illness, damage to self or personal belongings, injury, or death incurred from participation on the LA Rocks Summer Camp program.
